Probation Self-Assessment

Set a goal of getting off probation...now.  

The first step is to think about what got you here in the first place.  The self-assessment walks you through a reflection on what happened and guides you to strategies that may be helpful for you to be successful this time. 

The assessment takes about 10 minutes.  All responses are reviewed by the counselor and are kept confidential.  Complete this form and bring it with you when you meet with the counsleor to discuss your success plan.
